Welcome to the CPainMC. The
following information will help you navigate and get the most out of
your appointments.
New Patient Instructions
Please remember to bring all forms completed and signed to your first
appointment. Please review all documents under "Relevant Information
for Patients" prior to your appointment. If you need to cancel an
appointment, we require a 24 hour notice.
Please bring the following items with you to your appointment:
1. All insurance information
(including worker’s comp info).
2. Your authorization if you have an HMO insurance plan.
3. Any X-Rays, MRIs, or CT scan reports and films.
4. Latest progress notes from your referring physician.
5. Any other relevant information. |
